P R I V A C Y  P O L I C Y

 

1.

This Privacy Policy determines the rules of collecting, processing and using personal data acquired by www.medquest.eu  Website (hereinafter referred to as: ”Website”).

 

2.

The Website owner and concurrently the Personal Data Administrator is Grażyna Polkowska, who conducts her business activity under the business name of POINT-OF-CARE Grażyna Polkowska with registered office in Warszawa (01-494), ul. Andrzeja Sołtana 12 lok. 62, entered into the Central Register and Information on Economic Activity operated by the Minister of Economy, NIP (Taxpayer Identification Number): 5222927303, REGON (National Business Registry Number): 143184940, hereinafter referred to as POINT-OF-CARE.

 

3.

POINT-OF-CARE shall exercise due diligence to respect the privacy of Users visiting the Website.

 

4.

All definitions included in this Privacy Policy refer to the Website Terms and Conditions available on www.medquest.eu website.

§ 1 How do we collect data?

 

1.

POINT-OF-CARE collects information about natural persons entering into legal transactions not related directly to their activity, natural persons conducting the business or professional activity in his/her own name and natural persons representing any legal persons or organizational units not being legal persons that have been granted legal capacity by law, conducting the business or professional activity in their own name.

 

2.

The Users’ personal data are collected in the case of Registration process in the Website by creating an Account or using the services provided by electronic means via Website.

 

3.

In the event of Registration of Accunt in the Website, the User provides the following data:

 

a)

e-mail address;

 

b)

name and surname;

 

c)

telephone numbe;

 

d)

address (street, house/flat number, postcode and city);

 

4.

In the case of Entrepreneurs, the additional data is to be provided:

 

a)

Entrepreneur’s company;

 

b)

NIP (Taxpayer Identification Number);

 

c)

REGON (National Business Registry Number);

 

d)

fax number;

 

e)

name and surname of authorised recipient of VAT invoice.

 

5.

In the event of using the contact form in the Website, the User provides the following data:

 

a)

name and surname;

 

b)

e-mail address.

 

6.

In the event of expressing the will to use the Newsletter service, the User provides only his/her e-mail address.

 

7.

During the use of Website pages, the additional data are collected (IP address assigned to your computer or external IP address of Internet service provider, domain name, browser type, access time, operating system type).

 

8.

The navigation data may be also collected from the Users, including information about the links and references, on which the Users have clicked or other activities performed in our Website.

§ 2 How do we use the collected data?

 

1.

POINT-OF-CARE uses the collected personal data in order to implement the services provided by electronic means via Website.

 

2.

In the event of concluding the reservation Agreement, the User’s personal data shall be transferred to the Organizer, with whom the User has concluded the agreement.

 

3.

In the event when the User has paid for the concluded reservation Agreement via PayU system, his/her personal data shall be transferred in the scope required for implementation of agreement to PayU S.A. company with registered office in Poznań.

 

4.

The navigation data may be used in order to provide the Users with better service, statistical data analysis and to adapt the Website to the Users’ preferences, to administer the Website, and also to adapt the advertisements displayed on the Website pages to the Users.

§ 3 Cookies mechanism, IP address

 

1.

The Website uses small files called cookies. They are saved by POINT-OF-CARE via Website on the computer of person visiting the Website, if the Internet browser enables it. The cookie file usually consists of the name of domain, from which it comes from, its “expiration time” and individual, randomly selected number identifying the file. The information collected with the use of files of this type helps to adapt the services offered by POINT-OF-CARE to the individual preferences and actual needs of persons visiting the Website. This also gives the possibility to develop general statistics of displays of information provided in the Website.

 

2.

POINT-OF-CARE uses two types of cookies files:

 

a)

session cookies: after the end of session of particular browser or turning off the computer, the saved information are removed from the device memory. The mechanism of session cookies does not allow to download any personal data or any other confidential information from the Users’ computer;

 

b)

persistent cookies: are stored on the User’s computer hard disk and remain there until they are deleted.

 

3.

POINT-OF-CARE uses external cookies in order to:

 

a)

collect general and anonymous statistical data by means of Google Analytics tools (administrator of external cookies: Google Inc with registered office in USA);

 

b)

present multimedia contents on the Website pages, which are download from the external www.youtube.com website (administrator of external cookies: Google Inc with registered office in USA);

 

c)

Popularize the Website via twitter.com social networking site (administrator of external cookies: Twitter Inc. with registered office in USA);

 

d)

present the Certificate of Reliable Terms and Conditions via rzetelnyregulamin.pl website (administrator of external cookies: B2B Consulting Sp. z o.o. with registered office in Warszawa).

The mechanism of cookies is safe for the computers of Website Users. In particular, no viruses or other unwanted software or malware can enter the Users’ computers through the use of cookies. However, the Users have the possibility to limit or disable the access of cookies to their computers. In the event of selecting this option, the use of Website will be possible, except for the functions, which by their nature require cookies.

Below we show you how to change the settings of Internet browsers in the scope of using the cookies files:

POINT-OF-CARE may collect your IP addresses. The IP address is a number assigned to the computer of person visiting the Website by the Internet service provider. The IP number enables the access to the Internet. In most cases it is assigned to the computer dynamically, i.e. it changes at each Internet connection and due to that it is commonly considered as non-personal identifying information. The IP address is used by POINT-OF-CARE during the diagnosis of technical problems with the server, development of statistical analysis (e.g. determination of regions with the highest ratio of visits), as information that is useful at the process of Website administration and improvement, and also for the purposes of security and possible identification of overloading the server, unsolicited automated programs for reviewing the Website content.

 

7.

The Website contains the links and references to other websites. POINT-OF-CARE shall not be responsible for the privacy principles that are applicable on these websites. For that reason, we encourage you to review the privacy policy accepted on the website that you visit through this Website and which is administered by other administrator.

§ 4 Access to data

 

1.

The personal data collected by the Website are accessed directly only by the authorized workers and co-workers of POINT-OF-CARE.

 

2.

Your data may be also accessed by the entities authorized to receive them under the provisions of applicable law, in particular, relevant judicial authorities and entities indicated in the section: “How do we use the collected data?”

 

3.

POINT-OF-CARE provides the User, who have the Account, with uninterrupted access to his/her data and possibility of their modification at any time. In order to modify, the User needs to log in to the Account in the Website. The User may remove his/her personal data from the data set in the event of termination of agreement on provision of services by electronic means. POINT-OF-CARE may refuse to remove the User’s personal data in the event when the User has violated the Terms and Conditions  applicable in the Website, and keeping the personal data is necessary to explain such circumstances and establish the liability, and in particular in the event of pursuing claims against the individual User.

 

4.

In the event of newsletter subscription, you may remove your e-mail address from the mailing list by selecting the appropriate link sent contained in each e-mail message sent within the Newsletter.

§ 5 Security measures

 

1.

POINT-OF-CARE uses security measures that provide data protection against loss, misuse and alteration. POINT-OF-CARE shall undertake to protect all your data in accordance with standards of security and confidentiality.

 

2.

The personal data in POINT-OF-CARE are protected in accordance with the Act of 29 August 1997 on Personal Data Protection (consolidated text: Journal of Laws of 2002, No. 101, Item 926, as amended) in such a manner as to prevent the access of third parties.

 

3.

In the event when the User, who has an Account in the Website, has lost the access password in any manner, the Website allows for generating a new password to the Account in the Website. However, the Website does not send a password remainder. The User’s password is stored in the database in the encrypted form that prevents its reading. In order to generate the new password, the e-mail address needs to be included in the form, which is available under “Forget your password” link, which is provided next to the Account login form in the Website. The new password will be send automatically only to the e-mail address provided during the Registration process or saved during the last profile modification.

 

4.

POINT-OF-CARE shall never send any correspondence, including the e-mail messages requesting the login data, and in particular, the access password to the User Account. Such information may also not available by means of traditional, e-mail or telephone communication.

§ 6 Amendments to Privacy Policy

 

1.

POINT-OF-CARE reserves the right to introduce amendments to the Privacy Policy. On the date of introducing the amendments,  we update the date of last modification that is set below.
